ABSTRACT

Introducción: La miocardiopatía por estrés o síndrome de Takotsubo está asociado a la reducción de la fracción de eyección, niveles elevados de enzimas cardiacas y signos de isquemia en el electrocardiograma. Sin embargo, en pocas ocasiones trascurre con complicaciones tan importantes como el taponamiento cardiaco, deterioro significativo de la contractilidad y la posibilidad de trombos dentro de la cavidad ventricular. Estas son subestimadas a pesar de poner en riesgo la vida del paciente. Objetivo: Describir el caso de una paciente con miocardiopatía por estrés con taponamiento cardiaco como una complicación inusual. Caso clínico: Paciente en la novena década de la vida con antecedente de hipertensión arterial sistémica primaria y enfermedad pulmonar obstructiva crónica. Presentó dolor precordial con trastornos de la contractilidad de patrón usual correspondiente a la cardiomiopatía de Takotsubo en la que se descartaron otros diagnósticos diferenciales. Conclusiones: El caso presentado de miocardiopatía de Takotsubo asociado a una complicación cardiovascular tan importante e inusual como el taponamiento cardiaco, aporta información sobre esta enfermedad infrecuente en nuestro medio(AU)


Introduction: Stress cardiomyopathy or Takotsubo syndrome is associated with reduced ejection fraction, elevated levels of cardiac enzymes and signs of ischemia on the electrocardiogram. However, it rarely occurs with complications as important as cardiac tamponade, significant deterioration of contractility and the possibility of thrombi within the ventricular cavity. These are underestimated despite putting the patient's life at risk. Objective: To describe the case of a patient with stress cardiomyopathy with cardiac tamponade as an unusual complication. Clinical case report: A case of a female patient in her nineties is reported due to her history of primary systemic arterial hypertension and chronic obstructive pulmonary disease. She had chest pain with contractility disorders of the usual pattern corresponding to Takotsubo cardiomyopathy in which other differential diagnoses were ruled out. Conclusions: This case of Takotsubo cardiomyopathy associated with a cardiovascular complication as important and unusual as cardiac tamponade, provides information on this rare disease in our setting(AU)

ABSTRACT

Abstract Background There has been an increase in the number of cases of Takotsubo syndrome (TTS) and of scientific publications on the theme over the last years. However, little is known about the status of this disease in Brazilian hospitals. Objective To assess mortality and major adverse cardiovascular events (MACE) during hospitalization and follow-up of TTS patients seen in a tertiary hospital in Brazil. Methods This was a retrospective, observational study on 48 patients. Clinical data, signs and symptoms, complementary tests, MACE and all-cause mortality were assessed on admission and during follow-up. Kaplan-Meier curves were used for analysis of all-cause mortality and risk for MACE at median follow-up. The 95% confidence interval was also calculated for a significance level of 5%. Results Mean age of patients was 71 years (SD±13 years), and most patients were women (n=41; 85.4%). During hospitalization, four patients (8.3%) died and five (10.4%) developed MACE. At median follow-up of 354.5 days (IQR of 81.5-896.5 days), the risk of all-cause mortality and MACE was 11.1% (95% CI= 1.8-20.3%) and 12.7% (95% CI= 3.3-22.3%), respectively. Conclusion TTS was associated with high morbidity and mortality rates in a tertiary hospital in Brazil, which were comparable to those observed in acute coronary syndrome. ABSTRACT

RESUMEN Con el objetivo de describir las características clínicas y complicaciones del síndrome de Takotsubo, se realizó un estudio de serie de casos de pacientes que ingresaron con esta patología al Instituto Nacional Cardiovascular-INCOR en Lima-Perú, entre enero de 2013 a diciembre de 2018. Se incluyeron 26 pacientes, con una edad promedio de 69 años y predominio del sexo femenino (96,2%), además un desencadenante se identificó en 23 casos (88,5%). En el electrocardiograma, el 61,5% tuvo supradesnivel del segmento ST; y en la evolución el 92,3% mostró ondas T negativas y el 38,5% un intervalo QTc >500 ms. Las complicaciones intrahospitalarias fueron choque cardiogénico (11,5%), fibrilación auricular (7,7%) y taquicardia ventricular (7,7%). En esta serie, el síndrome de Takotsubo predominó en mujeres posmenopáusicas, generalmente desencadenado por un factor estresante, con una baja tasa de complicaciones y ausencia de mortalidad intrahospitalaria.


ABSTRACT In order to describe the clinical features and complications of Takotsubo syndrome, a case series study was conducted with patients admitted with this pathology to the National Cardiovascular Institute-INCOR in Lima-Peru between January 2013 and December 2018. Twenty-six patients (26) were included, with an average age of 69 years and female predominance (96.2%); additionally, a trigger was identified in 23 cases (88.5%). In the electrocardiogram, 61.5% had ST segment elevation; and, in the evolution, 92.3% showed negative T waves and 38.5% a QTc interval >500 ms. In-hospital complications were cardiogenic shock (11.5%), atrial fibrillation (7.7%) and ventricular tachycardia (7.7%). In this series, Takotsubo syndrome predominated in postmenopausal women, usually triggered by a stressor, with a low complication rate and no in-hospital mortality.

ABSTRACT

Most patients diagnosed with takotsubo cardiomyopathies are expected to almost completely recover, and their prognosis is excellent. However, complications can occur in the acute phase. We present a case of a woman with takotsubo cardiomyopathy with right ventricular involvement who developed a rupture of the right ventricular free wall following ventricular septal rupture, as a consequence of an acute increase in right ventricular afterload by left-to-right shunt. Our case report illustrates that takotsubo cardiomyopathy can be life threatening in the acute phase. Ventricular septal rupture in biventricular takotsubo cardiomyopathy may be a harbinger of cardiac tamponade by right ventricular rupture.

ABSTRACT

Fundamento: A cardiomiopatia de estresse/Takotsubo (CT) é uma entidade diagnóstica cada vez mais reconhecida. Objetivo: Este estudo teve como objetivo avaliar a prevalência e os preditores clínicos de complicações de curto e longo prazo de pacientes (pts) com CT. Métodos: Foram incluídos todos os pts consecutivamente admitidos no nosso centro, entre novembro de 2006 e agosto de 2011, que preenchiam os critérios diagnósticos da Clínica Mayo. Resultados: Analisaram-se 37 pts (35 mulheres), com idade média de 63 ± 13 anos. A CT foi precipitada na maioria dos casos por eventos de estresse emocional (57%) e dor torácica foi o sintoma de apresentação mais frequente (89%). O electrocardiograma na admissão mostrou supradesnivelamento do segmento ST em 12 pts (32%) e inversão da onda T em 15 casos (41%). Verificou-se disfunção sistólica ventricular esquerda (VE) grave em 16 pts (45%) e a elevação média de troponina I foi de 2,6 ± 1,8 ng/mL. A taxa de complicações intra-hospitalares foi de 30%, sendo o choque cardiogênico a situação mais comum. O estresse físico, a disfunção sistólica grave do VE e o valor de pico do peptídeo natriurético cerebral (BNP) foram preditores de complicações agudas. Não foi encontrada associação entre o pico de troponina I e a apresentação eletrocardiográfica. Trinta e cinco pacientes foram acompanhados por um tempo médio de 482 ± 512 dias, sem recorrência clínica. Conclusão: Na nossa série de pacientes, a CT foi associada a uma alta taxa de complicações intra-hospitalares. O estresse físico, a disfunção sistólica do VE e o valor de pico do BNP foram preditores de desfechos adversos agudos. .


Background: Stress-induced/Takotsubo cardiomyopathy (TC) is an increasingly recognized diagnostic entity. Objective: This study was aimed to assess the prevalence and clinical predictors of short and long-term outcome of patients (pts) diagnosed with TC. Methods: We included all consecutive pts admitted in our department, from November 2006 to August 2011, who met the Mayo Clinic criteria for TC diagnosis. Results: We evaluated 37 pts (35 women) with a mean age of 63 ±13 years. TC was precipitated by a stressful emotional event in the majority (57%) and chest pain was the most common symptom (89%). Twelve pts (32%) had ST-segment elevation and 15 pts (41%) had T-wave inversion on the electrocardiogram at admission. Severe left ventricular (LV) dysfunction was found in 16 pts (43%) and the mean troponin I level was 2.6±1.8 ng/mL. The in-hospital complication rate was 30%, with cardiogenic shock being the most common situation. Physical stress, severe LV systolic dysfunction and peak brain natriuretic peptide (BNP) were predictors of acute complications. On the other hand, we found no association between peak troponin I and electrocardiographic presentation. Thirty-five pts were followed for a mean time of 482 ± 512 days, without clinic recurrence. Conclusion: In our cohort of pts, TC was associated with a high in-hospital complications rate. Physical stress, LV dysfunction and peak BNP could predict acute adverse outcomes. .

ABSTRACT

La cardiomiopatía por estrés (de Takotsubo) es un síndrome caracterizado por disfunción ventricular izquierda transitoria, con anomalías de la contractilidad características, predominantemente a nivel apical. Presentamos el caso de un niño de 2 años quién desarrolló una cardiomiopatía de Takotsubo después del cierre quirúrgico de una comunicación interventricular. Este síndrome es poco común en niños, y a nuestro conocimiento, este es el primer caso de este tipo de cardiomiopatía tras cirugía cardíaca en un niño.

ABSTRACT

Medically refractory ventricular tachycardia (VT) storm can be controlled with radiofrequency catheter ablation (RFCA), however, it may be difficult to control in some patients with hemodynamic overload. We experienced a patient with intractable VT storm controlled by hemodynamic unloading. The patient had mid-septal hypertrophic cardiomyopathy with an implantable cardioverter defibrillator (ICD) back-up. Because of the severe mid-septal hypertrophy, his left ventricle (LV) had an hourglass-like morphology and showed apical ballooning; the focus of VT was at the border of apical ballooning. Although we performed VT ablation because of electrical storm with multiple ICD shocks, VT recurred 1 hour after procedure. As the post-RFCA monomorphic VT was refractory to anti-tachycardia pacing or ICD shock, we reduced the hemodynamic overload of LV with beta-blockade, hydration, and sedation. VT spontaneously stopped 1.5 hours later and the patient has remained free of VT for 24 months with beta-blockade alone. In patients with VT storm refractory to antiarrhythmic drugs or RFCA, the mechanism of mechano-electrical feedback should be considered and hemodynamic unloading may be an essential component of treatment.

ABSTRACT

A cardiomiopatia induzida por estresse é uma síndrome que mimetiza o infarto agudo do miocárdio. A presença de doença arterial coronária significativa é considerada um critério de exclusão de cardiomiopatia induzida por estresse; contudo, isso pode não ocorrer em todos os casos, dado que ambas as entidades podem coexistir. Apresentamos o relato de um caso de cardiomiopatia induzida por estresse em paciente com doença arterial coronária conhecida.


Stress-induced cardiomyopathy mimics acute myocardial infarction. Significant coronary artery disease is generally considered an exclusion criterion for the diagnosis of stress-induced cardiomyopathy. However, this may not be the case for all patients, since both entities may coexist. We present a case of stress-induced cardiomyopathy in a patient with known coronary artery disease.

ABSTRACT

Classicamente, a cardiomiopatia de Takotsubo caracteriza-se por alteração transitória da contratilidade regional envolvendo o ápex e os segmentos médios do ventrículo esquerdo, com segmentos basais hipercontráteis, na ausência de doença coronária obstrutiva. Recentemente, casos de disfunção regional do ventrículo esquerdo induzidos pelo estresse, com padrões alternativos de disfunção regional em que o ápex é geralmente poupado (Takotsubo Invertido), têm sido relatados. Neste relato é descrito um caso de miocardiopatia induzida pelo estresse, no qual alterações da contratilidade segmentar foram observadas somente nos segmentos médios dos ventrículos, formando uma região acinética em faixa em torno do coração.

ABSTRACT

Prolongation of QTc interval associated with Takotsubo cardiomyopathy (TC) has previously been reported in published case series. We report an unusual case of a patient who presented with TC associated with long-QT syndrome and developed cardiac arrest secondary to torsade de pointes. Since QT prolongation and bradycardia persisted after the resolution of TC, the patient received permanent pacemaker. Since then additional event did not occur. QT prolongation and bradycardia could be persistent even after recovery of TC, and permanent pacemaker insertion may be a treatment option of long QT syndrome related with TC.

ABSTRACT

Acute and rapidly reversible left ventricular dysfunction may be triggered by various psychological and physical insults. This entity is now well known as stress cardiomyopathy or Takotsubu cardiomyopathy. Suicidal hanging involves intense emotional outburst and the act of hanging is an extreme physical stress. We report a case of rapidly reversible left ventricular dysfunction following attempted suicide by hanging. Usually the outcome is favourable as in our case. The pathogenesis of left ventricular dysfunction in hanging is myocardial stunning due to catecholamine surge. Other mechanisms are also proposed.

ABSTRACT

Apical ballooning syndrome [ABS] is a reversible cardiomyopathy with presentation mimicking an acute coronary syndrome. So in clinical practice, it is essential to consider it in the differential diagnosis of patients presenting with chest pain, especially in postmenopausal women. Coronary angiography is usually indicated to achieve a proper diagnosis. Typically, patients do not have significant coronary artery lesions. Left ventriculography and echocardiography reveal a regional systolic dysfunction with akinesis of the midventricle, apex and compensatory hyperkinesis of the basal ventricular segments. Occurrence of an emotionally or physically stressful event is a feature of ABS but its absence does not exclude this diagnosis. Several pathophysiologic mechanisms had been proposed. The prognosis of ABS is good. In this review, we highlight the clinical manifestations, pathophysiology and management of this syndrome
